CALD Community Care Program (CCCP)
The CALD Community Care Program (CCCP) is a support, information and referral service for Culturally & Linguistically Diverse (CALD) older people, people with disabilities and their carers living in the St George and Sutherland areas. The CCCP serves as a point of entry into the community care system for CALD clients by providing:
The CCCP can also provide advice, support and training to HACC service providers aimed and making the HACC service system more responsive to the needs of the CALD HACC target population – Strengthening Capacity.

St George MRC – CALD Community Care Program has recently produced two resources to help clients and service providers improve access and communication processes

The Hospital and Community Care Guide for CALD Carers is a practical guide for informal family carers aimed at providing easily accessible, easy to read, practical information on how to navigate the hospital system. Available in PDF or CD format, it is translated into ten languages reflecting the demographics of the local population. It contains information on what to expect whilst in hospital; the role of different health professionals; interpreter services; post-discharge planning; and information about local community care services. The information is presented in simple, straightforward and easy to understand language. Languages available: English, Arabic, Chinese, Greek, Italian, Macedonian, Serbian, Bosnian, Croatian, Spanish and Hindi.
The Communication Pictorial Tool Kit - was developed for use by Community Care service providers as a communication tool when working with care recipients from CALD backgrounds or care recipients who are non verbal. The Kit is designed to prompt and facilitate communication and has been translated into 14 community languages. Languages available: Arabic, Chinese, Greek, Italian, Macedonian, Serbian, Bosnian, Croatian, Spanish, Filipino/Tagalog, Indonesian, German and Vietnamese.

The Multicultural Community Aged Care Package (CACP)
Multicultural Community Aged Care Packages (M-CACP) is a service of St. George Migrant Resource Centre. The service provides culturally and linguistically appropriate support for the elderly people of Non English Speaking Background who live in St George areas and Sutherland Shires. The services are individually planned and coordinated packages of care tailored to help clients remain living in their own homes and aim to prevent early admission into nursing home or hostel for support. This project is funded by the Commonwealth Government through the Department of Health and Ageing to provide for the complex care needs of older people.
What Service is provided?
The services CACP provides include:
Personal Care:
An appropriately trained staff will assist a person to shower; this will consist of assisting the person into the bathroom and ensuring that the person is safely in the shower. The staff member can assist with dental care or washing the hair for example. Further, the person is then assisted with dressing and general grooming.
Staff will then make you comfortable and make you a cup of tea whilst they make the bathroom safe by collecting towels and clothing or moping bathroom floor if it is too wet, the bathroom has to be made safe to use by others.
In some cases some minor modifications may have to be done to the bathroom before a worker can do these tasks, a shower chair for example may be required if you are unsteady on your feet or are not able to stand for too longer periods.
Home Help:
CACP is not a cleaning service, our staff will attend to simple cleaning tasks that are in some cases difficult for clients to do, cleaning the bathroom, vacuuming and laundry and ironing of two pieces clothing per week for example.
Shopping and Banking:
The CACP program is able to assist people with their shopping and banking by providing a one to one service to ensure safety, CACP staff will accompany clients on their shopping and banking trips.
Transport:
Transport is limited to the St. George and Sutherland LGA only and is provided by our staff using their own vehicles. Transport is provided for shopping and banking and medical appointments, currently the fee for transport is $ 0.60 cents per Km.
Case Management:
The St. George MRC CACP program has language specific Case Managers attached to its packages; Case management is to assist clients with information and advocacy, assist clients liaise with other parties like GP’s, Housing NSW or other service providers for example. Case Managers will monitor a person’s general wellbeing and will adjust the service to meet each individual’s changing needs.
Who is eligible for the service?
Older people from Non English Speaking Backgrounds who live in their own home or with family can use this service. To be eligible persons must live in the St. George area and Sutherland Shires, and must be assessed for suitability by the local Aged Care Assessment Team (ACAT).
How can CACP help me?
When your assessment has been approved for CACP services, you will be assessed by a CACP Coordinator, then a Case Manager will be appointed to plan and manage your care package, tailoring it to your individual needs. For example, a package may give you assistance with personal care such as bathing and dressing, domestic assistance such as housework and shopping, or possibly help participating in social activities. All services are provided by trained bilingual staff, speaking your language and understanding your cultural needs.
How can I access a CACP?
To receive a CACP, you must be assessed by an Aged Care Assessment Team (ACAT) as requiring at least low level care. ACAT assessments are free of charge. If M-CACPs are available in your area, you have been assessed as needing low level care, and expressed a preference to be cared for in your home, then you may be offered a CACP package of care.
